Community Planning
The Community Planning Working Council (CPWC) is a coalition of Government and Non-Government agencies as well as consumers and concerned members of the community. The mission of the CPWC is to assure that our Community has the assets and resources necessary to provide the broad range of prevention, medical and support services needed by Persons at Risk for infection.
The CPWC is a project of the Fresno County Department of Community Health (FCDCH). Stephanie Garcia

A program to stop the unintentional spread of HIV by persons living with HIV by providing them with information, counseling and resources that will encourage them to notify their sex and/or needle/syringe sharing partners of their potential risk for HIV infection, either directly or through provider- assisted methods.
Main goal is to provide services to HIV infected persons and their partners so they can avoid infection, or if already infected, can prevent transmission to others.
Services include partners gaining earlier access to individualized counseling, HIV testing, medical evaluation treatment and other prevention services. This is a voluntary, confidential service that is client-centered and on going. We offer supportive services and referrals.
Program Description:
Westcare is a comprehensive prevention, treatment, and rehabilitation community based programs. West Care has been serving chemically dependent individuals and their families for more than twenty years. The HIV component provides education and prevention to men who have sex with men, injection drug users, and the African American Gay/MSM Action Plan.
The African American Gay/Men/MSM Action Plan
An Action Plan to respond to the disproportionate impact of HIV/AIDS on African American gay men/MSM in Fresno County. Staff will identify the health needs/priorities, determine knowledge and attitude with regards to HIV through various sources (including individual interviews, focus groups and needs assessment). The long term goal is to provide effective HIV prevention services for AA gay men/MSM. The action plan will assist in identifying possible opportunities and solutions to reach the targeted population.
Injection Drug User
Provides Education and Prevention Information to clients who are at high risk for HIV transmission. THe program includes both individual and group sessions. Outreach sites are provided at or around known sites of needle exchange an include the distribution of risk reduction packets.
Men Who Have Sex with Men
This program serves Men Who Have Sex with Men. They perform outreach in various locations. Outreach packets are distributed. Outreach packets include condoms, lube, and testing information. Group sessions are held using evidence based curriculum. They participate in local health fairs. An internet site has been established for person seeking information on sex education.
