Kern County HIV/AIDS Education & Prevention Program: The Kern County Department of Public Health in conjunction with three subcontractors – Clinica Sierra Vista, Community Action Partnership of Kern, and Planned Parenthood Mar Monte target Kern’s population of over 662,000 implementing HIV education and risk reduction interventions to high-risk individuals at different levels of intervention (individual, group, outreach, and targeted activities) to reduce the transmission of HIV. Kern County reported 1,745 AIDS Cases as of 12/26/07 and 774 HIV cases as of 12/26/07.

Kern County Ryan White CARE Act Title II Program: The Kern County Department of Public Health works with its in-house Early Intervention and Case Management Programs (EIP/CMP), and Clinica Sierra Vista/Kern Lifeline as subcontractors to provide the following services: Medical Care (ambulatory, dental and vision care), Substance Abuse Treatment Services, HIV antibody testing, Partner Counseling and Referral Services (PCRS), AIDS Drug Assistance Program (ADAP), and linkages to other services/support services.

Kern County Housing Opportunities for People With AIDS (HOPWA) Program: The Kern County Department of Public Health in collaboration with three subcontractors – Clinica Sierra Vista/Kern Lifeline, Kern County Dept. of Public Health’s EIP/CMP programs, and Housing Authority of the County of Kern provide Tenant Based Rental Assistance Housing and Supportive Services to clients living with HIV/AIDS. Supportive services include: adult day care and/or personal assistance, substance abuse services, case management/client advocacy/access to benefits and services, child care/other child services, employment assistance and training, education/prevention with positives, health/medical and intensive care services, legal services, transportation, meals/nutrition services, outreach and life skills management.

Kern County HIV Transitional Case Management Program (TCMP): The Kern County Department of Public Health provides transitional services from prison to the community for parolees living with HIV/AIDS in Kern, Kings, and Tulare Counties. The following services are provided through referrals and linkages: housing, medical/dental care, food/transportation, substance abuse referrals, educational programs, public benefits/SSI-Ga, employment services, mental health services, support groups, HIV/AIDS/TB and Hepatitis education.

Kern County HIV/AIDS Integration of Services/Access to Services: The Kern County Dept. of Public Health through inter-agency collaboration with subcontractors and different programs within the Department provide clients access to available services through different ports of entry such as – Sexually Transmitted Diseases (STD) Clinics, Family Planning/Prenatal Care Clinics, TB Control Clinics/Program, Black Infant Health Program, HOPWA/Ryan White CARE Act Title II programs, Medical Care, Case Management and Support Services, HIV Counseling and Testing/PCRS Programs, Substance Abuse Treatment Services, and HIV Education and Prevention Services.

CAPK operates an intervention program based on the Mpowerment Project and serves men who have sex with men, and focuses particular emphasis on young MSM between 18 and 35 years of age.
