Santa Clara County, also referred to as "Silicon Valley", is the largest county in the San Francisco Bay area and the fourth largest in the state of California. It comprises 1,312 square miles populated by more than 1.7 million residents in fifteen cities, including San Jose and unincorporated areas. While a significant portion of the County's land area is unincorporated forestland, 92% of the populations lives in cities. The County employs a work force of 16,000. Best known for semiconductor, Internet, and information technology and home of the world famous Gilroy Garlic Festival, held every July.

Nurses and social workers provide assessment of clients with disabling HIV/AIDS. Nurses, social workers, case managers and benefits counselors coordinate in-home support and connect individuals and families with community services and resources, disability insurance, benefits counseling, medical care, mental health counseling, nutritional counseling, health and hospice care, mental health counseling, nutritional counseling, health and hospice care plus enrollment in the AIDS research protocol. Assistance with finding subsidized housing. Practical support includes houseing and rental assistance, food vouchers and Meals on Wheels, emergency funds for utilities, management of attendant care, homemaker services, referrals for respite care, and assistance in acquiring social services. Update, accurate HIV disease information. Limited burial funds available. Community education on special needs and issues involving persons living with HIV/AIDS.
